I know I was to lazy to change mine for a very long time. It is either that or they cant seem to figure out how (It is very easy to do so I don't see why the wouldn't .-.)
Basically. Many people who get avatars are people who want to make a name for themselves or be otherwise distinguished from the crowd. Since 80%-ish of any website is lurkers, most people only post a few comments, and only more active ones invest some customization in their profile

Life is not measured in the breaths we take, but by the moments that take our breath away.
I can agree I had the default for maybe first hundred posts I made. Then seeing how active I was on the forum I decided to use a avatar so I can be noticed among the other users. Why do they use the default? I wouldn't know but being lazy can be one. Another is they don't feel like it as I have seen some users with the default that have high post counts. Some may don't want to be known as others.
I think that's part of it, laziness. To say for myself I had the default avatar because the pics I wanted were usually too big, and then trying to resize or crop would just reset it for some reason, and somehow it would always get taken off.. it was just a worthless frustration. But maybe I'm just part of the minority.
This is actually the only forum I frequent where the default avatar is so... well... dumb. Ideally avatars should default to not existing at all.
It's not really laziness, but some people don't think they are important. And they aren't. A lot of people also identify people by their avatars, too; which has some merit, in that a avatar might be more easily recognized than a name, (to the point where you see people walking around confused and scared when a long-standing member or somebody who has never changed their avatar changes it).
For some it could be considered "laziness" but that implies that it's better to have an avatar.

Lots of people have never been to a forum before until they've seen the epic Minecraft Forums!

Some people do not know how to change their avatar.
Another reason would be they do not know what they want their avatar to be.
