First off, the scene starts in a natural underground structure, most likely a cave or a stronghold.
After throwing some torches down, using flint and steel, and exploring a bit, something happens. Many people presume it's a mob, or Herobrine. I'm not being so quick to assume it's him yet, so let's say it's a mob.
If it is a mob, it doesn't make an audible sound. So it must be a Creeper, because Endermen do make noises, even when not provoked.
If the mob was a Creeper, the player doesn't fight, but runs away. Then sprints. We know he doesn't fight the Creeper because there is no hissing, and it doesn't explode. By now we can assume the player got away, the steps are made from stone to grass.
Then the record cuts off. This could simply be an explanation of the broken record, that there's no more to it. Nevertheless, it is a spooky ending.
If it was Herobrine, which I don't recommend believing in, we could infer that the player had found him in the cave/stronghold. And fled like mad, but might not have gotten away.
Sincerely, who knows? It could simply be a track to scare players and keep them on their toes. What do you think?

I would say he is in a stronghold because notice how the transition from stone to grass footsteps takes place... constant fast steps on stone to constant fast steps on grass... there was no stopping to jump
So this means 3 things
the guy was coming out of a cave where the exit was a flat surface
the guy built stairs or something in the exit of a cave
or
the guy was coming out of a stronghold due to their being stairs at the exit (now i have never seen a stronghold in my own personal games so I don't know how you enter or exit one so forgive me if I am wrong)
It could just be something to keep players on their toes but if it is a stronghold he is exiting it could possibly hint towards a new mob in the stronghold or the boss mob?

so we can probably eliminate those two possibilities.
Most likely, it's just an extremely creepy audio track, reminiscent of a "chase".
Although, I believe I can hear Ghast sounds. May just be my imagination.
idk. the minecraft wiki says the person on the disc is attacked by an enderman. you can even hear "it" right at the end before it cuts off. this could be somewhat of a preview to what they sound like.

The music file for the disc has been in the game files a long time before notch even came up with the idea of Endermen. No new music files were added in this update. Just unused ones that are now attached to music records.
The 'song' has been in existence LONG before it was added to the game. General consensus was that he's running through the nether, I think. It definitely can't be Endermen: Those weren't even thought of at the time.
Does nobody at all remember when this track was first released? It's funny how people are making totally different conclusions about this song than they did when it first came out, shortly after the Halloween update I believe. People then thought it was related to the nether, hence the ghast noises.
Actually, I'm going to stretch things a little. And I don't mean C418 actually took any influence from this or anything, but it reminds me of something quite Lovecraftian.
"The Music of Erich Zann," if anyone has ever read it. I think that may fit. It is an unidentifiable horror, because it is a horror we are not meant to know. It's entirely psychological. It's things beyond our native perceptions. In a way, that's what I imagine. Something you're not even sure you saw, but you know it is there.
Perhaps to up the creepiness even more, imaginatively, perhaps even the cave is like the street Rue d'Auseil. You find it once. But never again.
