Hmm.. i have a lot of them.. guess ill post the most recent.
I was playing on easy, and for the start i made a nice little dirt house and a tunnel leading to a deep cave next to it. I opened the door to outside and punched down some trees at night, then i went back inside but dumbass me forgot to shut the door. What do you know? 4 creepers got inside and suicide bombed my precious house. There was nothing left D:
I kept everything of importance with me, and went into my really deep cave, trying to make this giant cavern lit up. Once a creeper came from the light and exploded, and then out of the light a zombie came and killed me. I had found 2 of the 5 diamonds I had ever found in indev or infdev, and a lot of other things, including my first ever redstone, and I was way too far away to get any of it before it disappeared. Now I have learned not to keep everything with me.

It may not seem much now, but soon that will seem like a lot.
Its like going to a police station telling them how you stole a purse and demonstrating it, while arguing the benefits of it. You'd still go to jail because you're an idiot.
I spent 95 hours building the most epic and fantastically badass looking fortress right into the side of snowy mountain. My blood, sweat and tears went into this thing. Towers, walls, cart tracks, a mine deeper than 3 grand canyons, stacks of , both records (green and gold), and a few monster farming places I dig with my BARE HANDS! This was and forever will be my most epic creation known... EVER.
All of the map, all 20+Mb was all taken away overnight by a a blue screen of death that left my computer naked and alone.
Just thinking of that map makes me cry...
I hadn't found diamond yet as i explored natural caves instead of shafting to the expensive stuff. So i went to this SMP server and in lack of unexplored natural caves i made a shaft under my house. Ended up in a dark natural cave and was happy and thrilled until i tried to melt my first 30 iron bars. Customed to singleplayer i ofcourse left the crafting screen loosing all of it and all my coal as well. Rather annoyed i carried on and suddenly i stumbled across a diamond, i was all exited and rushed back to my furnance to make myself a iron pick. This time only putting in one coal and one iron, but the process would stop at 50%'ish for some reason and the iron would never melt. After asking for help or to trade some iron i eventualy gave up and asked if someone would trade my 20'ish unmelted iron for 3 iron bars. When i finaly got someone willing to trade i quickly made a iron pick, dug up the 4 diamonds, gave them to the one that traded me and quit the server.
Enritched with the knowlege of diamond i finaly found some on my singleplayer save, got a find of 3 and another of 4, but as i got greedy i ended up beeing killed by a surprise skeleton making me run into the arms of a loving creeper. I wheren't able to recover any items.
After that i always litter my caves with chests and small stations so that i never carry large amounts of valuables for long and if the amount gets big enoug in the chests i do a test run to the surface to ensure that it is safe even though i know i'd be able to handle any loose spawns that might appear.
Tl:dr: I lost my first pieces of diamond so now im paranoid and use chests heavily.
i had a bucket of lava in my hand and i right clicked to open my wooden door to my wooden house that was 3 floors high then after the lava killed me and i spawned halfway across the map lets just say i use stone now
I was in a cave and noticed that the entire ceiling was sand. I noted that I should be careful not to touch anything, then I saw a mushroom and forgot what I was just thinking and picked it up. The ceiling promptly collapsed and suffocated me. All because of an F'ing mushroom.

As I was going to store it in my chest I made down in my excavation site I right clicked the chest while I was holding my bucket and it automatically spilled the lava all over, burning me and ultimately killing me.
The respawn area was miles away from where I died and I lost a great deal of ore from that accident.
Let's just say it was enough ore to make anyone cringe a bit.

My epic fail?
I had a boosted minecart, and a creeper was in it. Anyways, he triggers the switch and goes down into my underground base, explodes at the front, triggers all of my TNT traps, and completely destroyed it.
Whazzat noise? I hear zombie! But I don't see zombie... there must be a dungeon nearby, cuz I hear more than just one zombie! :ohmy.gif:

Also, on a related note, stay away from water when you're in lava country. I know it sounds contradictory, but you'll thank me when you can recover your inventory when a mob kills you instead of your items floating out to magma on damned currents.
I never even got to mine the obsidian with the diamonds I found... D:
I wad building a lava moat around my base and i accidentally got caught on fire and started running around and accidentally ran into one of my wood buildings and caught it on fire which was right next to my tree farm which burned half of my place down :sad.gif:

I was playing in a dark room just before bed and was deep underground in a natural cave. I was trying to walk a narrow bridge over a rather large lava pool and paused the game to take a sip of water and then because the room was dark, when I restarted I wasnt coordinated on the keyboard, so hit Q rather than W to try and walk forward... and threw my last diamond pickaxe into the lava.
I had been saving that pickaxe because I've only found like, 6 diamond in the two months I've been on this world... so it was barely used. The tsss it made haunts me still.

Fortunately enough, I was playing on easy, so I got away with 3 hearts but...that was pretty stupid of me.
