Not sure if this is a bug, because I didn't actually witness it happening.
I spent some time down in my mine shaft directly below my keep. Upon returning to the surface, I found a and a chilling on the second floor of my keep. Now, I quickly retreated downstairs and checked all the doors and found them still closed. There's no way they could have spawned in that room on their own because it is very well lit, but it's the only explanation I can think of.
Not sure if anyone else has had this issue since the update, but if I shoot a bow and arrow at a monster it just falls to the ground at the monsters feet and deals no damage.
EDIT: I rolled my server back to .0.2.4 until the bugs get fixed and now I cannot connect to my server with the 1.2.3 client tells me client out of date is this a normal or am I missing something.
I believe out of date client means out of date server. As far as I know, because your server is linked with the game server (over which you have no control) your only choice is to run the most up to date version. This is why your server goes down with Notch updates.
My post regarding bugs was deleted so I'll post it here because some of the things I encountered are not listed here. I've been using minecarts successfully for a couple weeks up until the last update (well last two). Now as soon as I right click on one it crashes the game. Didn't see this in the official buglist, just the minecarts going invisible thing which I haven't had a chance to encounter. I also fall from the sky every time I spawn and lose all but 1/2 a health point.

Only affected by 1 bug so far. But it's a big one.
No matter what, I can't pick anything up. My inventory is clear, I've never died, the server and client have both been restarted multiple times, it's been happening since the update (so it's not lag), and as a result, have lost 9 diamonds and a diamond pickaxe.
Thanks to this update, the game is essentially unplayable now. I can only hope that notch releases a bugfix soon.
Looking into this more, it is probably because I am invisible. I don't know how or why I'm invisible, as I've never died and I've got my difficulty set as normal.
All the "dying and not dying" problems are probably because the difficulty setting is (and always has been) client side. Thus, if it's not on normal, the server has a different opinion on what's hit you and your health, than your client.
You can't see the monsters when in peaceful mode, because peaceful doesn't have monsters.
Best bet at the moment is to leave difficulty on normal. I would presume at some point that difficulty will become a single server-side option, not client side. It doesn't make sense for some people to have monsters and others not to.
Alright, I have the "stuck in the void" problem. I wasn't wearing armor but i did have the difficulty on peaceful. However, upon changing it back to normal, closing the browser and connecting to my server again, I still was in the void, invisible and unable to pick items up, with everything I broke dissapearing. Can you tell me how to fix this? anybody? I can't do anything until this is resolved....
Last night my girlfriend and I spent 3+ hours building a farmstead on our private SMP server. We log in today after the "update" and it was reset to the original rudimentary crate frame we had built.
After our initial dissapointment we mined all the required materials and went back to the site to begin anew. Everything seemed fine until we attempted to break blocks around the site, which proved invulnerable; including the crates we placed.
Along with the numerous bugs posted on this thread, it seems everything we interracted with the night before won't break. Also, we had some materials in our inventory from after we were done the previous night that we did not possess before construction on the farm(such as books)---thus proving that the loss of everything we built was not simply a complete "wipe".
We tried multiple things to get the blocks unstuck---resetting internet connection, logging in and out, and restarting the server.

This may or may not be the server-reverting problem, but a server will not save changes if it's window is just closed and not 'gracefully shut down' by using the 'stop' command in the console. I've lost a good bit of work before by accidentally just closing the window, which disregards all changes and keeps your world at the state it was last saved at.
However if you did use the stop command this may just be a bug. Be surety enable automatic saves anyways.
Conserning the not-being-able-to-remove-blocks problem, I had that a few times, and sadly don't know how to really fix it. It always seemed to go away after logging in and out and changing my difficulty setting, or restarting the server.

I forgot to mention in the previous post that I run an hMod server!
What I did was madk a copy of the map dubbed "world", deleted the whole server and its folder, then download the hMod server again and pasted the copied world back into the folder. This made the blocks breakable.
Hope this works for anyone else experiencing this problem. I'll be sure to make copies of my world and properly save before exiting now!
* If you reconnect immediately after dieing, you will keep your old inventory, but still could pickup items that were dropped on your death (you get double items).
Not completely sure if that happens always when you reconnect, but just happened to me.
I'm only able to pick up 2 wheat seeds out of several that appeared. After planting those and harvesting the wheat, I'm only able to pick up one seed out of the 4 that appeared.
Seems the EXE server (I can't comment on the Jar server as I don't get a fancy graph) has a memory leak:

No one has been on for hours. But over time Memory use has slowly gone up from 45 mb, to now 181 mb. When I woke up after a nap this afternoon, use was at 638 mb!
So, is there a memory leak? Something is, over time, causing my server to bloat, even though no one is on. This has -never- been an issue before. Normally memory levels are at their -lowest- when the server is idle. Now they are higher than they have ever been when I have had 8 players on at once.
Hope this can get addressed ASAP. Not sure how to make Notch aware of this.

Player death is very broken. When killed I drop all my stuff (minus the armor I am wearing) but I am still able to walk around, destroy blocks, use switches. I am however unable to pick anything up. Current workaround I use it to remove my armor and log out. Upon logging back in I finally get the respwan dialog and once respawned I can pickup items and be hurt again.

No damage sounds when hitting monsters or animals. No creeper "light the fuze" or explosion sound.
Damage via diamond sword seems to have been reduced significantly. It takes almost a dozen hits it seems to kill a zombie.
Another bug I seem to have a lot is item's duping or disappearing when moving around in the inventory, this is especially prevalent when moving items in chests or between chests and your own inventory. I don't hate when they dupe, but when you lose a chunk of diamonds because it flat out disappeared, RAGE lol.
Everyone on my server tries to have and keep the same difficutly setting as everyone else. Yet, each person seems to be experinces different errors with a few common for all.

Then there is the infinite arrow bug and the bug of arrows only sometimes hurting a mob

Mine carts and boosters work fine, as does red stone.
Wooden pressure plates usually work, but on some wooden doors they don't.
Also, some of the leaf blocks are invisiable on trees grown from saplings, until you disconnect and reconnect.
Some skeletons will attack and others won't.
Some mobs (usually zombies never skeletons) can hurt you from three or blocks away and through blocks sometimes.

I think that those are all the bugs that players on my server are experincing. No one player seems to be exerincing quite the same bugs as the others. Again, we all tend to play on the same difficulty so if it was an issue with bugs related to certain difficulty everyone should be experincing the same bugs, but they aren't.
List tags are malformed.
Your coal and the things you were about to smelt both disappear if you don't put them in at the same time, and the smelter doesn't even work when you do most of the time.
