He tries to relight it then the enderman finds him. he goes running out of the cave but the enderman catches up to him and it kills him

it was in 1.7.3 when the music was released into the files so notch had PLANS for endermen he was working on them no doubt
the video could have been around the time of 1.7.3 he just had an old version

Think about it. When a disk is messed up, what sound does it make? Static.
Also, if you listen closely (I got a Warn Status for posting about Herobrine before...) at the last scream, it kinda sounds like 'Herobri-' and then you reach the damaged part.

That music disc for some reason reminds me of Marble Hornets or something, i was thinking about something like that while listening to it.

The sound at the end is dead sound from a record, like the record that it is being played on.
I got to agree with you. The record isn't damaged. The static is that of the end of an audio cassette after the recording is finished. If it had been a damaged record we would hear scratches and jumps at the end not static.
I don't doubt that this isn't here to just freak you out. I think it may be foreshadowing something to be added secretly in a later update. (Why was the person recording though!?) Freaky? Yes. Significant? Yes. But what I'm wondering is why people have all sorts of theories about this, but none about the significance of Cave noises/ambiances.
It is definitely foreshadowing. Notch has mentioned several times that he intends to give Minecraft a storyline that you can either play or ignore before the game's official release in November. There is only a very short time left before then. So, it shouldn't be surprising to see snippets of possible story elements appearing in current and future updates.
Since it sounds like the book he was flipping through might have been left behind, it might be possible to find in a chest somewhere at some future date. Notch also mentioned in previous tweats and on his blog that he wanted us to be able to read and maybe write in books. At a future date we might be able to read the journal of the man of Disk 11.
The metallic sound is certainly flint and steel being flicked as if attempting to light something. If the man was in the Nether, maybe he was trying to re-light a Nether portal to escape back to the Overworld. We don't hear fire in the background or creature sounds. The man may not have been in the Nether at all. He could be in a cave or even above ground with all the passive mobs mysteriously absent. It is possible that he could have been running from a creeper. Other hostile mobs make sounds which would have been heard in the background. Even the Endermen make sounds.
The coughing doesn't have to have been from smoke inhalation. It could have been disease or old age. We can get poisoned now as of the 1.8 update. We might see illnesses later. Or maybe the person on the record is just very old and can't run like they could when he or she was young. The gender of the person on the recording is also unclear. We never hear them speak only run, cough, flip through some paper, and attempt to ignite something with flint and steel.
IT IS NOT HEROBRINE. THat sound at the end sounds more like howling wind than someone saying Herobrine. It doesn't even sound like speech.

This record is very, very creepy. I hope that more numbered records are released. Hopefully we will be able to make some connection between all of the numbered records.
I think i hear a motor at the end. Sounds like he was mining and lit a torch with flint and steel. Notch did say we would have to relight torches in the future some time. He sees something down his mineshaft perhaps a creeper Enderman silver fish or Herobrine or just another player who hunts him down or a new mob. He runs away and pulls out a map to find his way out of the cave. When he gets out he is sprinting and you can hear a motor like a helicopter.

Heard it in a Yogscast vid, as I have decided to not install 1.9 myself until it is officially released. Creeped me out, and Simon and Lewis were seemingly pretty creeped out too, though they could have just been acting I suppose.
11- You hear a man is running deep in a mine, he gasps for air as he falls on one knee. He opens his backpack for his torch but only to hear the clicking of a dead battery. Coughing in pain he covers his mouth only to find blood pooling in his hand. He reaches for a lighter and his diary of his excavation of the mines and begins to write as grotesque sounds echo throughout the mines. He slowly rises and uses his lighter trying to see out of the pitch dark as a shadowy figure slowly starts creeping in his direction, he sees a way out of the mines and runs without hesitation, he sees moonlight and sprints entering a patch of gravel, the exit gets closer and closer, mere inches from freedom when he screams in terror as he sees a………the recorder stops, the static from the broken disc send you chills down your spine and yet you feel as if something has been watching you ever since you began listen to the broken disc.
I made this when i was watching the Yogscast 1.9 video on music disc this is what i imagine when i hear it.
